I hereby release, discharge, and/or otherwise indemnify MC United Soccer, its officers and volunteers, including owners of the fields and facilities utilized for the programs. I understand that MC United, including its officers, coaches and referees, will not be held liable for any injury that may occur to the registrant while participating in the MC United Recreational Soccer League. I agree to provide health and accident insurance personally and will take care of any injury under my own policy. I have read the MC United parental code of conduct and agree to abide by the code. I also agree to review the player code of conduct with the above listed registrant prior to the start of the recreational soccer season. As the parent or guardian of the above registrant(s), I hereby give my consent for emergency medical treatment prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve life, limb or well being of my dependents(s). I give my consent to MC United to release the name and photograph or image of the above registered player to the media (newsletter, newspaper, web site, etc.) to promote the MC United and/or youth soccer. In addition, I permit MC United to share player address information with organizations running soccer programs that prepresent soccer opportunities for my players.
